The NWS Health and Wellness Services Fees Reimbursement Program was developed to assist and encourage employees to achieve and maintain physical fitness through the use of health club and wellness services, such as physical fitness maintenance/improvement, weight loss, or smoking cessation programs. Employees, enrolled in the program may request reimbursement, up to $300 per calendar year, for the expense of health club and wellness services. Monthly dues with proof of membership is allowable.

 

Reimbursement for 2017 Expenses

Employees enrolled in the Program during the 2017 calendar year should begin preparing and submitting requests for reimbursement to their supervisor. The Office of Human Capital Services exempted these requests for reimbursement from submission and processing in the HR Connect system. Accordingly, no later than March 16, employees must submit Form B, Self-Certification Usage (attached) and proof of payment for membership/services. Within 15 days of receipt of this documentation, the manager/supervisor will submit a completed CD-326, Recommendation for Recognition, for the approved reimbursable amount (up to a maximum of $300) to wfmo.pay@noaa.gov for processing. All versions of the CD-326 are acceptable, including the 2016 Version .[Managers: WFMO is requesting each reimbursement be submitted separately, for tracking purposes]. Proof of payment for membership/services and/or other supporting documentation must also be submitted. Please Note:  When completing the CD-326, the home portfolio rule must be followed. In other words, the health fee reimbursement should be charged to the same project code used for the employees’ time and attendance. Use task code PHK for Health Club Benefits Reimbursement. Contact your FMC Budget Officer if you have questions on the proper accounting.

 

EXCEPTION FOR REIMBURSABLE EMPLOYEES: If you normally charge your time and attendance to a reimbursable project code (e.g., CWSU employees), please contact your FMC budget officer for the appropriate project code to use for health club reimbursement. Do not charge this expense to a reimbursable project as such expenses are not authorized.

 

Enrollment for Calendar Year 2018

Open season for the 2018 Health and Wellness Services Fee Reimbursement Program is January 1 – March 16. To enroll, the employee must submit Form A, Health Club Reimbursement Application (attached), to his/her manager/supervisor by March 16, 2018. The manager/supervisor will sign and date Form A, acknowledging the employee's enrollment. In addition, the manager/supervisor will file all collected Form A documents from his/her staff in a central file.

 

New employees or existing employees unable to apply during the open season due to prolonged absences may apply within 14 calendar days of returning or entering service.
